Abstract
Background In vitro studies have demonstrated that direct oral anticoagulants (DOACs) downregulate expression of proteins involved in calcification and inflammation. In this hypothesis-testing study we evaluated whether DOAC therapy was associated with decreased valvular calcification in patients with aortic stenosis (AS) who were anticoagulated due to concomitant atrial fibrillation (AF). Methods In this case-control study, 72 Caucasian patients with isolated severe AS were compared with 53 individuals with AF concomitant to severe AS who were treated with DOACs for 28.7 ± 13.8 months. Sixteen valves from AS patients on rivaroxaban (20 mg/day, AS-RIVA group) and 20 valves from age- and sex-matched non-anticoagulated patients with AS were subjected to microcomputed tomography (micro-CT) to estimate valvular calcification ex vivo. Calcium volume (CV), surface volume (SV), CV/SV ratio, and trabecular thickness (TbTh) were assessed. Valvular expression of osteopontin, nuclear factor-kappaB (NF-κB), and interleukin-6 (IL-6) were evaluated by immunostaining. Results Micro-CT showed that patients taking rivaroxaban had a lower CV (−62.7%), lower SV (−46.2%), decreased CV/SV ratio (−35.6%), and maximal TbTh (−21.1%) when compared with patients not taking the drug (P < 0.05 for all). The duration of rivaroxaban use correlated inversely with micro-CT parameters, peak transvalvular velocity, and maximal transvalvular pressure gradient. Decreased valvular expression of osteopontin (−20.4%) and NF-κB (−26%) and 2-fold lower IL-6 fluorescence intensity were observed in the AS-RIVA group compared with the remainder (P < 0.05 for all). Moreover, osteopontin expression was inversely associated with duration of rivaroxaban use and positively with micro-CT parameters. Conclusions Long-term rivaroxaban use in AS patients was associated with lower aortic valve leaflet calcification, -as reflected by micro-CT parameters, and osteopontin expression, suggesting a potential impact of rivaroxaban on valvular calcification.
Abstract
Contexte Plusieurs études in vitro ont montré que les anticoagulants oraux directs (AOD) régulent à la baisse l’expression des protéines intervenant dans la calcification et l’inflammation. Cette étude de validation d’hypothèse vise à déterminer si le traitement par un AOD est associé à une diminution de la calcification valvulaire chez des patients présentant une sténose aortique (SA) qui reçoivent un anticoagulant pour une fibrillation auriculaire (FA) concomitante. Méthodologie Dans cette étude cas-témoins, 72 patients de race blanche présentant une SA sévère isolée ont été comparés à 53 patients présentant une SA sévère et une FA, traités par des AOD pendant 28,7 ± 13,8 mois. Seize valves de patients présentant une SA traités par le rivaroxaban (20 mg/jour) et 20 valves de patients présentant une SA qui ne recevaient aucun anticoagulant, appariés selon l’âge et le sexe, ont été soumises à une tomographie micro-informatisée pour estimer la calcification valvulaire ex vivo. Le volume de calcium (VC), le volume de surface (VS), le rapport VC/VS et l’épaisseur trabéculaire ont été évalués. L’expression valvulaire de l’ostéopontine, du facteur nucléaire κB (NF-κB) et de l’interleukine 6 (IL-6) a été évaluée par immunocoloration. Résultats La tomographie micro-informatisée a révélé que, chez les patients présentant une SA traités par le rivaroxaban, le VC (-62,7 %), le VS (-46,2 %), le rapport VC/VS (-35,6 %) et l’épaisseur trabéculaire maximale (-21,1 %) étaient moindres, comparativement aux patients qui ne recevaient pas de rivaroxaban (p < 0,05 dans tous les cas). Une corrélation inverse a été observée entre la durée d’utilisation du rivaroxaban et les paramètres de tomographie micro-informatisée, la vitesse maximale transvalvulaire et le gradient de pression transvalvulaire maximal. Une diminution de l’expression valvulaire de l’ostéopontine (-20,4 %) et du NF-κB (-26 %), et une intensité de fluorescence deux fois plus faible de l’IL-6 ont été observées dans le groupe présentant une SA traité par le rivaroxaban comparativement aux autres patients (p < 0,05 dans tous les cas). Un lien inverse a par ailleurs été établi entre l’expression de l’ostéopontine et la durée d’utilisation du rivaroxaban, tandis qu’une association positive a été observée entre l’expression de l’ostéopontine et les paramètres de tomographie micro-informatisée. Conclusions L’utilisation de rivaroxaban sur une longue période chez les patients présentant une SA a été associée à une diminution de la calcification du feuillet de la valve aortique, d’après les paramètres de tomographie micro-informatisée, et de l’expression de l’ostéopontine, ce qui donne à penser que ce médicament pourrait avoir un effet sur la calcification valvulaire.
